Before entering a lottery, you need to visit with a lender and have them complete this form. They will attach a credit report, and you will then submit it along with the Lottery Entry Sheet and your Homebuyer Education Certificate to be entered into a lottery.

Your Lender's Qualification Worksheet will be held on file with TCHA for one year. After a year, a new one will need to be submitted. The Homebuyer Education Certificate is good indefinately.
